Job Description
The Memory Care Coordinator is the main point of contact for residents, families, and staff regarding resident care needs for our Footsteps Memory Care Program. The MCC must be an excellent communicator as they need to ensure all parties are appropriately informed. In this position, you will work closely with the community RN to ensure the accuracy of care for our treasured residents. Duties include:
  • Oversight of Footsteps Memory Care Program
  • Maintaining accurate resident service plans
  • Monitoring alert charting and communication logs
  • Assuring our team of Caregivers and Med Techs are appropriately trained and supported, including helping to work the floor as needed, and providing coaching and mentoring
  • Performance management and staff development
  • Collaborating with life enrichment staff on planning activity programs for Memory Care
  • Health Services staff scheduling (in conjunction with RSC)
Qualifications:
  • 5 years demonstrated experience successfully leading teams
  • Professional experience working with senior populations with dementia
  • Prior experience working in senior living as RSC
  • Prior experience with medication administration and caregiving highly preferred
  • Ability to pass criminal background and pre-employment drug test
  • 3-5 years Health Care experience
  • MC and OARS regulations knowledge

  • Sincere appreciation for elders and observable respect for caregivers, med techs and the important work they do
  • Ability to effectively present information and respond to questions from groups of managers, residents, employees, and the general public.
  • Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations
  • Ability to write reports, business correspondence, and procedure manuals.  Ability to prioritize, organize, and delegate assignments.
  • Ability to interpret a variety of instructions furnished in written, oral, electronic, or schedule form.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
This position is part of the greater health services team supporting senior residents in assisted living and memory care. The team includes the Memory Care Coordinator and Resident Services Coordinators, Assisted Living Resident Services Coordinators, RN Director of Health Services, and Health Care Administrator.
