The Springs at The Waterfront is situated along the southern facing Vancouver riverfront and will provide approximately 250 private residences for those 62 and older. Job Description:
The Springs at The Waterfront is looking for a compassionate and dedicated Memory Care Coordinator is committed to providing excellent care and attention to those we serve. This position reports to the Health Services Administrator. As an MCC in our community, you function as the main point of contact for residents, families, and staff in regard to care needs, and oversee the entirety of the Footsteps Memory Care program. You will work closely with the community Health Services Administrator, RN and LPN to ensure accuracy of care for our treasured residents. Other responsibilities include oversight of service planning, assistance with employee scheduling, and team leadership.
Responsibilities:
- Oversee daily execution of health services team tasks, including but not limited to ADL's, medication administration, care plan creation, TSP's, completion of new resident binders and paperwork, etc.
- Monitoring alert charting and communication logs, monitoring and filing incident reports
- Create, monitor, and update all resident care plans, working closely with the nursing team
- Informing Health Services team members of changes in resident care or status
- Communicating with doctors offices and other health team providers, scheduling visits as needed
- Leading and conducting regular care conferences with residents and their families
- Oversight of health services team members, including coaching and mentoring
- Overseeing and creating health services schedule for caregivers and med techs, covering for shifts as needed
- Helps lead the health services team to ensure daily tasks are completed
Qualifications:
- 3 years previous experience in health services leadership role required, preferably in Memory Care
- Active WA State NAC or Home Care Aide certifications
- WA Nurse Delegation Certificate
- Prior experience as Med Tech and/or caregiver preferred
- Demonstrated ability to lead a team
- Demonstrated ability to prioritize tasks and meet hard deadlines
- Expert knowledge on providing care in a senior living setting, including WA regulations knowledge
- Ability to effectively present information and respond to questions from groups of managers, residents, employees, and the general public
- Ability to prioritize, organize, and delegate assignments
- Memory Care regulations knowledge
- Ability to interpret a variety of instructions furnished in written, oral, electronic, or schedule form
- Proficient PC skills including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.

Salary:
The Springs Living provides a competitive compensation package for the Memory Care Coordinator position, with an hourly wage of $26.00 - $28.50 per hour, depending on experience. This position may be also eligible for a year end discretionary bonus.
