The Memory Care Resident Services Coordinator (RSC) assures all service issues of memory care residents are effectively addressed as directed by the Memory Care Administrator or the Health Services Administrator, as well as performing certain duties related to employee health and safety.
Responsibilities Include:
- Serves as a primary point of contact for memory care residents or residents' families regarding resident care.
- Assists in conducting continuing education and training for health services employees and other employees as directed by the Memory Care Administrator or Health Services Administrator.
- Monitors Communication Log, assuring health services employees are informed of changes and enters pertinent information as necessary.
- Advises health services employees concerning resident issues and directs employees to notify RN, Memory Care Administrator, and or Health Services Administrator as necessary.
- Finds replacement health services employees in case of open shifts (if unable to find a replacement employee) will be responsible for covering the open shift.
- Creates new resident charts and ensures they are updated, organized, and purged according to The Springs Living policy.
- Enters all new residents' personal information into Point Click Care, ensuring all information is accurate and that new residents are on alert charting and monthly vitals are assigned.
- Performs chart audits to ensure each chart complies with state regulations. (This also includes POLST form reviews, ensuring all are up to date and are placed in each chart and resident apartment).
- Assists in counseling employees as needed.
- Meet routinely with residents and their families to review their care plan to ensure accuracy and that it reflects their needs and preferences.
- Shift Info: Tuesday - Saturday OR Sunday - Thursday
- Wage Info: Starts at $21 per hour
Skills:
- Proficiency in understanding and implementing resident rights and regulatory requirements.
- Ability to administer medications accurately and safely.
- Knowledge of social work principles to support residents' emotional well-being.
- Leadership skills to guide a team of health services employees effectively.
- Expertise in creating comprehensive care plans tailored to individual needs.
- Proven ability in supervising and managing staff to provide high-quality care.
Qualifications:
- Medication Technician Experience required.
- Experience in health care services and familiarity with healthcare practices required.
- Ability to effectively present information and respond to questions from groups or managers, residents, employees, and the general public.
- Maintain current CPR/first Aid certification.
- Ability to prioritize, organize, and delegate assignments.
- Ability to interpret a variety of instructions furnished in written, oral, electronic, or schedule form.
- Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Assisted Living and Memory Care Knowledge.
- Proficient PC skills including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.
- Involves light to heavy physical activity, performing non-strenuous and strenuous daily activities.
- Ability to pass a criminal background check and drug test
