Job Description:
We are seeking a passionate and driven On-Call Physical Therapist to deliver high-quality in- house therapy services to adults with a range of diagnoses and disabilities, with a special focus on supporting aging in place. In this role, you will directly influence residents' quality of life by providing person-centered interventions that promote mobility, independence, and overall well-being. Primary Responsibilities:
- Conducts screenings of new community residents and those referred by nursing to determine the need for intervention
- Evaluates residents to obtain data necessary for treatment, planning and implementation
- Interprets and communicates evaluation findings to residents, family, and other team members
- Records treatment times and documentation by following federal and state laws, rules, and regulations
- Demonstrates capacity to understand insurance reimbursement and billing while coordinating care
- Provides treatment in accordance with established standards of practice, department procedures and productivity standards while adapting treatment for individual needs
- Monitors resident response to intervention, modifying treatment as indicated to attain goals in accordance with physician orders
- Recommends and educates residents, family, and community staff in the use of adaptive equipment and assistive devices
- Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs including excellent communication with community staff
- Maintains safety standards including safety stands including compliance with infection control, universal precautions and OSHA stands for healthcare professionals
Qualifications:
- Graduate of an accredited therapy program - Physical Therapist (PT)
- Current state licensure
- Excellent verbal, written and interpersonal communication skills
- Results-oriented with strong critical thinking and problem-solving skills
